The Florida Department of Health in Suwannee County provides School Health services with a School Health Coordinator RN, an RN, an LPN and one School Health Aide. These staff supplement the health services provided by Suwannee County School District Staff.

School Health Services include:

  • Medication Administration
  • Health Education
  • Health Counseling
  • Referrals
  • Routine Health Screenings
  • Nursing Consults with Students and Parents
  • Assessments of Illnesses and Injuries

School Entry Requirements 

Physical Exams- For first-time entry in Florida Schools (K-12) a physical examination within the past 12 months is required.  Florida Statute (S.1003.22(1) F.S.), Florida Administrative Code (6A-6.024 F.A.C.)

Immunizations- Florida Certification of Immunization, Form 680 must be used for entry and attendance in Florida Schools.  Florida Statute (S. 1003.22(4), F.S.)

School Entry Vaccination Requirements- Florida Administrative Code (64D-3.046, F.A.C.)

Public/Non-Public Schools Kindergarten through 12th Grade:

  • Four or five doses of diphtheria-tetanus-pertussis (DTaP vaccine)
  • Two or three doses of Hepatitis B (Hep B vaccine)
  • Three, four or five doses of polio vaccine*
  • Two doses of measles-mumps-rubella (MMR) vaccine
  • Two doses of varicella vaccine ** for kindergarten and grades one through five
  • One dose of varicella vaccine ** for grades six through twelve

*If the fourth dose of vaccine is administered prior to the fourth birthday, a fifth dose of polio vaccine is required for kindergarten.

**Varicella vaccine is not required if varicella disease is documented by the healthcare provider.

Seventh Grade Requirements:

In addition to kindergarten through 12th grade requirements, students must have the following vaccinations:

  • One dose of tetanus-diphtheria-pertussis (Tdap) vaccine in grades seven through eleven
