Community Health Needs Assessment Survey

The Florida Department of Health in Lafayette and Suwannee Counties, in partnership with WellFlorida Council, the local health planning council for North Florida, are encouraging community members to share their views on health by taking a 10-minute survey. The surveys are available at www.WellFlorida.org (select “Survey”). Participants will be entered to win a $20 gift card.

“This survey is very important to our communities,” states Mr. Kerry S. Waldron, Administrator for the Department of Health in Lafayette and Suwannee Counties. “Information obtained concerning the health issues in our communities will be the driving force in developing strategies and objectives for addressing high priority health areas over the next 3-5 years.”

Survey results, along with research and health-related data, will be used to assess the region’s most pressing health needs. This information will be made available in January in the report Lafayette and Suwannee County Community Health Needs Assessment 2018.
